There are just over two weeks before the 2021 State Aid Application Filing deadline on February 28, 2021. Please note that due to COVID-19, there were no changes to the required forms for 2021. We wanted to remind clients to email the 2021 Application, as well as the forms, to the Office of Public Health Practice (a6fis@health.ny.gov) and any applicable personnel from your regional office by the deadline.
